1. Authorization for Assistance: By completing and submitting this maintenance request form, I, the undersigned owner of the specified unit, grant Eaglerock Property Management CC authorization to provide assistance for the aforementioned maintenance request. 2. Financial Accountability: a. I understand and accept that I bear responsibility for any expenses not covered by the Body Corporate/HOA as outlined in compliance with the STMA, 2011 (Act No.8 of 2011) and/or Articles of Association. b. Prior to the commencement of maintenance, I agree to settle any additional amounts owed to the insurance or contractor beyond the coverage limit. 3. Insurance Claim Settlement: a. I commit to covering any outstanding expenses not covered by the insurance claim associated with the maintenance request. b. I affirm that I have thoroughly examined the scheme insurance policy and comprehended the limitations of coverage provided. 4. Liability Acknowledgment: a. I acknowledge that Eaglerock Property Management CC and the Body Corporate/HOA disclaim responsibility for any damages incurred. It remains my responsibility as the owner to take reasonable precautions to prevent further damages. 5. Claims Completion Responsibility: a. I acknowledge that it is my duty as the owner to complete and promptly return the essential insurance claim form to Eaglerock Property Management CC for necessary assistance. b. Failure to complete the claim and furnish the required documentation may result in the rejection of the claim. In such cases, I, as the owner, accept liability for the entire cost of the claim. 6. Completion of Form Agreement: a. I recognize that failure to fully complete and submit this form will preclude Eaglerock Property Management CC or the Body Corporate/HOA from extending assistance regarding this maintenance request.
